AKC FILLABLE ENTRY FORM A copy of your entry(ies) payment transaction will be send directly to the Show Secretary as confirmation of payment. The Show Secretary will receive confirmation of payment for entries and ads. Please fill out the Entry Form and email to wideview@ptd.net along with your confirmation of payment number. If you have any problems with this process - then email wideview@ptd.net for help or phone 908-454-6456. *****NOTE*** Entry form has been revised to include "Additional Classes".
